Position Title: Director of Coordinated Housing Placement

Supervised By: Executive Director

Position Type: Full Time; Exempt

Job Description:

The Director of Coordinated Housing Placement is responsible for coordinating, monitoring, and overseeing key elements of the Continuum of Care's (CoC) Coordinated Entry process to house veterans, families, and individuals experiencing chronic homelessness. Work will be supported and informed by the Homeless Management Information System, (HMIS), which is administered by the Coalition.

Coordinated Entry is a data-driven concept widely accepted as best practice in homeless assistance systems to achieve three goals:

  1. Helping people move through the homeless system faster
  2. Reducing new entries into homelessness
  3. Improving data collection and quality

Primary to the work will be operationalizing the CoC Placement Committees: Veteran, Chronic Homelessness, and Youth and Family placement, bringing together homeless service and housing providers as well as community partners. The goals of these committees are: to place individuals and families in housing; and, identify barriers to placement in housing for resolution at the policy (administrative/legislative) and inter-agency collaboration/cooperation levels.

In addition to placement committee facilitation, the Director of Coordinated Housing Placement convenes. facilitates. and advises systems and policy improvements through other forums and coordinating committees designed to achieve the goals set out above.

Responsibilities and Accountabilities:

Housing Placement Committees

  • Staff all Housing Placement Committees (Vets, Chronic, Youth and Families) and related coordinaton grpups within the homeless service system
  • Promote and support Housing First and other effective national best practice models of Coordinated Entry and housing of people experiencing homelessness
  • Research and improve referral processes and policies to more quickly and accurately make referrals to housing. Implement them with the Vets, Chronic, and Youth and Family placement meetings and use data to monitor their efficacy
  • Establish mechanisms for ongoing feedback to plan needed changes and/or adjustments to Coordinated Entry and Housing Placement to ensure clarity, transparency, consistency and accountability for homeless clients, referral sources and homeless service providers throughout the process

Manage Continuum of Care's By Name Lists for Veterans, Families, and People experiencing Chronic Homelessness

  • Manage and Coordinate the CoC's "By Name Placement Lists" that support each committee's work
  • Adhere to and train others on proper privacy protocols
  • Support the development of and create HMIS practices and procedures in developing and managing the By Name Lists
  • Aid in development, promotion, and implementation of "move-on" strategies for those no longer in need of supportive housing to create more space for those entering the system

Continuum of Care Support

  • Promote the training and education of Coordinated Entry and Housing Placement model throughout the CoC
  • Problem solving (at Executive, Senior Staff levels) on collaboration issues that arise between and among service providers, state departments, and state governance committees
  • Help in the establishment of performance-based measurement systems to evaluate effectiveness, identify problems, and devise revisions in the Coordinated Entry System
  • Provide information and participate in other committees as needed to strengthen the CoC's Coordinated Entry system and related processes.

Other Duties as Assigned

Required Qualifications:

  • Passion for ending homelessness and Housing First principles
  • Ability to form and nurture relationships with a variety of stakeholders including people experiencing homelessness, front line service providers and outreach workers, executive and program directors, directors and staff of state agencies and the Continuum of Care
  • Exceptional organizational skills, multi-tasking abilities and attention to detail
  • Extensive experience in developing systems and collaboration through data driven and human focused work
  • Engaging and outgoing communicator and facilitator
  • Bachelors Degree
  • Reliable transportation and valid driver's license
  • Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint) including the ability to develop charts, and graphs

Preferred Qualifications:

  • Master's Degree in relevant field
  • Advanced knowledge of the homeless service system in Rhode Island
  • Working knowledge of the Homeless Management Information System (HMIS)
  • Experience in data analysis and reporting, program evaluation, and/or outcome measurement
  • A sense of humor and inquisitive nature

This position is grant funded and continuation of the position is contingent upon the continued availability of funding.
